Mandatory Requirements
A Birth Certificate OR

Vouching Process
The applicant must choose a relative who has been issued with a Ghana Card to identify him or her under oath before a commissioner for oaths.
Where there is no relative to vouch for the applicant, two persons approved by the Board who have already registered and have the Ghana Card can vouch for the applicant under oath.
Secondary Requirement
An applicant with the following valid national IDs like Drivers license, NHIA card, SSNIT number, GRA Tax Identification Number(TIN), Voter ID card, Passport must bring them along for harmonization of data.
